Course Title:

Web-based Certified Quality Auditor Refresher Course

Overview:

ASQ Section 509 is pleased to offer an online refresher course for quality professionals who want to prepare for the Quality Auditor Certification (CQA) Exam administered by the American Society for Quality (ASQ). The course covers the CQA Body of Knowledge and sample exam questions for you to test your understanding of the concepts. The course will help participants identify topics they should concentrate their studies on as they prepare for the actual exam. Levels of cognition will be reviewed and their use in defining the intended complexity of multiple choice test questions will be identified.

Format:

Web-based, online, course using AnyMeeting. Participants will need access to the internet. Use of a headset and camera are strongly encouraged.

Timing:

The course runs for 5 Saturdays from Saturday, April 23rd through Saturday, May 21st and will consist of 4, 5-hour and 1, 4-hour sessions. Each session will hold from 8:00 AM to 1:30 PM or 8:00 AM to 12:00 PM EST. All sessions will be recorded and available for viewing to participants until the exam date (06/04/2016).

Instructor Bio:

This is the fifth year, Ms. Menzies has taught the CQA Refresher course for ASQ Section 509. Ms. Menzies has been a senior member of ASQ since 2001. With over 24 years of experience, Ms. Menzies has held quality-related positions at Biocon, Inc.; Otsuka Maryland Research Institute; TherImmune Research Corporation; and the Food and Drug Administration (FDA). Ms. Menzies current position uses her knowledge of auditing and project management to improve internal business processes at the FDA. In addition to auditing for compliance to Good Clinical Practice and Good Laboratory Practice regulations and ISO 9001 and ISO 17025 standards, Ms. Menzies has hosted FDA and EPA inspections, and ISO audits.

Course Materials:

We will be using certification materials from the Quality Council of Indiana. Required materials for the course are: �CQA Primer, 8th edition (Jun, 2012) - $70 � Solution Text - $35 � CQA Exam CD - $70. The solutions text and CD-ROM from the Quality Council of Indiana are optional yet recommended. Note: These costs do not include tax, shipping, or handling. Exam Date:

Saturday, June 4, 2016. Exam date is Saturday, June 4, 2016. Application deadline is April 15, 2016. Late Application deadline is April 20, 2016 (includes $50 late fee).

ASQ CQA Exam Cost: $419 for non-members, $269 for members.  Join ASQ and save! Exam applicants are responsible for meeting ASQs certification requirements.

Exam Location:

Exam test location is in the Maryland suburbs near Washington, D.C. Applicants will be notified by National ASQ of their exam location and test time.

Course Withdrawal Policy:

The policy for the Web-based CQA course starting April 23, 2016: If you need to cancel your registration or withdraw from the course, we will refund your paid registration fee as follows: 1. Requests for cancellations/withdrawal received by April 11, 2016, receive a full refund. 2. Requests received after April 11, 2016, incurs a 25% processing fee. 3. There are no refunds past April 20, 2016.
